The phrase gets used for three different things that behave nothing alike. Only one of them involves two cameras being on at the same time.
The literal reading is the correct one
Face to face means your face and their face, at the same time, each visible to the other. Offline nobody needs this explained. Online the phrase got borrowed by products where it is not true — a page with one camera running and a chat box underneath will still call itself face to face, because the phrase sells something that the format does not deliver.
The test is simple and it has one question: is the other person's camera on too? If the answer is no, whatever you are doing has a different name.

Why the count of cameras changes the conversation
